Support Coordination

Support Coordination is included in some people’s NDIS Plans.

A Support Coordinator helps you with your plan and connects you to different services in the community. They focus on helping you learn new skills, make choices, and connect with people and providers who meet your needs.

Your Support Coordinator will also talk to people and providers to arrange what they can offer you and how much it will cost from your plan. They make sure all the paperwork is done, so you can use the services you need.

They help you become more independent, coordinate your support, and join in with your local community. They can also assist you in planning for your next plan review.

Support Coordinators are there to help you get the most out of the supports in your plan.
