Feedback & Complaints

Equity Works welcomes your feedback on our services.  At any time you can submit a complaint, provide a compliment or offer a suggestion to help us improve our service.  There are a few ways you can do this. See below.

  • We encourage people to provide feedback through this brief survey, conducted using Microsoft Forms. Your responses will be sent directly to Steph Whelan, Equity Works CEO.

  • Improvement Idea Form

    Anyone can use this form to suggest ideas that support high-quality and safer services for people we support. We want to make it as easy as possible for people to share their ideas on how we can improve our services.

    Your ideas will be shared with the CEO and the Management Committee and could be added to the Equity Works Continuous Improvement Plan.

  • If you have any concerns about the services you have received from us, we want to be the first people you tell.

    We would ask you to contact your usual Equity Works Contact, or if you do not feel comfortable with that option you can contact the Equity Works Office on 07 5441 2122 and ask to speak to the Chief Executive Officer or email s.whelan@equityworks.org

Complaints about your experience, privacy breach, or our handling of your personal information can be made verbally or in writing. Once we receive a complaint an Equity Works representative will be in contact with you within two (2) working days.

Our Complaints Management process focuses on listening well to the issues that are concerning you, seeking your advice about how those issues can be resolved to your satisfaction, doing our best to resolve the issues that have concerned you and finally, learning from the experience.

Anytime we receive a complaint, we will provide detailed information about our Complaints Management Process.

If you are not satisfied with our response or do not feel you can talk to us directly, you may lodge a complaint with the National Disability Insurance Scheme (NDIS) Quality and Safeguards Commission.
